Do I have to setup a Recipient List before I create the survey?

If you do not have the Recipient List already set up in AssociationSphere, you can create it on the Attach Recipient List step of the Survey Wizard. The same functionality exists here that exists on the Recipient List menu choice – you can import or type in a list directly on this step.
